Azalea Saotome dying! :(

I haven't really posted in this forum at all since I joined up, but when I get some more cash, I hope to get some more plants and be more proactive, but at this moment I'm just trying to keep my first tree alive!
A couple of weeks ago, my Azalea was knocked out of its pot and we put it back in with some more soil and put some fertiliser on it, but since then it looks as though it's slowly dying. Leaves have been turning brown and falling off and I'm afraid I've murdered it!
Can someone tell me what I can do to help it, or if it's totally condemned? I really don't know anything about gardening at all (amateur hour!), so some pointers would be great! I've managed to maintain it for almost a year, so on my terms, I thought that was pretty good.
